Anyone can rename a file that they uploaded themselves, even after the file is live. Done this several times myself, both before and after autoupload. Just go to user CP, click approved files, find the file and click edit.

needs a new cover and to fix the thumb nail preview you must delete and re-add

RH is correct, to change the contents of the file like adding a cover one needs to re-upload it.  If you change the file using Edit within User_CP / Approved Files - the thumbnail will not be updated.
If it was just a matter of renaming the actual file as it appears when you download it, only I can do that without needing to re-upload it.  But in this case it sounds like RH has more to do than just rename it.

and both times it wasn't the issue! WOW

Seems to be common in the Chesler Titles.

BUT,  I believe we could get these all scanned.
