Are you asking about patching a paper page scan with part of a fiche page?
If yes, here is what I do.

Open each page in PS.  On the paper page needing the fix duplicate the background in a new layer.
With the magic wand left click on the white 'hole' and then hit delete.
Now on the fiche page using the marquee highlight the section you need to copy over and be sure to give yourself some extra around it preferable something with a word balloon or panel edges.
Type 'ctl-c'
On the paper page type 'ctrl-v' to paste the fiche selection into the paper page as a new layer.
Use Transform and resize the fiche layer until it is close to the same size as the paper page.
Now on the layers window move Opacity down to around 40% so you can see through the fiche layer to the paper.
Move the fiche layer over the missing hole and position it using the extra word balloon type or panel boarders so the fiche layer overlaps the paper exactly.  Use Transform to resize exactly or rotate if needed.  Once they are exactly overlapping go back and change the Opacity to 100% again.
Now move the fiche layer down between the duplicate layer and the background layer.
You should now see the fiche only in the hole section.
Once you are happy - flatten the layers and save-as a jpg.

Hope that helps.  I use this technique quite a bit for fiche fixes.
