Hey, exactly what setting did you use on Irfanview? Mine end up pretty huge on 100, but perhaps I shouldn't be disabling chroma color subsampling.
Anyway, I'm pretty sure you're missing a page there. Between 5! and 6! there should be this:

And "From the Library of" page appears to have been the back of "page 2" rather than the inner front cover (which seems to have just been the first text page), so possibly unnecessary if you're dropping blank pages.

I've added the page. It was going to fall after pg2 with your file name so I tweaked it.
I don't know if Neon Vincent frequents DCM. The file was found floating on the net and was added here under his name.
