My scanner wasn't quite big enough to get the whole image.  Fred (and I'm ashamed I can't remember his last name!) gave this to me several years ago, it's one of the treasures of my collection.

(Hmm, apparently I can only see the attachment if I'm logged in.  Can anybody else see it?)
